I live in an industrial city and we don't have gasification yet. And it is located in an excavation. The air used to be very dirty, but recently the government has started to monitor the situation and to regulate emissions more strictly and to force large enterprises to upgrade their equipment and reduce emissions into the atomosphere. In general, things have gotten much better.
Air humidity, but only during the winter, because in the region where I live (Brazil), the winter is very dry, sometimes with desert indices.

Not anymore. The average in Delhi is around 100+ AQI.

I don't, but I have air purificator and I ventilate rooms whenever I can.
